I actually know this one, we have a lad doing his referee courses at my sons football club.
Usually because they are in conversation on their ear piece/mic with their nearest linesman. They are told to pause after blowing up to evaluate what’s happened, how severe and to make sure things aren’t going to escalate by them wading in before they act.
