Chain "jumping" off rear cog (Sturmey-Archer IGH + Surly singleator)

Preface: I have a Surly singleator that I lost the bushing for, so I used bushings from a chain which are the exact width (but short). So first question is, is the bushing in the singleator supposed to be slightly longer than the pulley width?



The issue I am having is my chain is "jumping" under load from the singleator to the cog, causing horrendous chain crunch. I'm wondering if I fubared the singleator so it is causing this somehow? I don't see how though as it's lined up perfectly.
