The Palace one was a problem as they apparently said 'yes we are actively running both clubs' whereas other clubs have shown different people were running each one in their multi club model.
I remember at the time it was said that it didn't take much to circumvent the rule but for whatever reason Palace owners didn't realise and said the wrong thing.
It's all a bit daft really, you just need to change some paperwork to say you don't have the same I guess parent company operating both teams and you avoid any issues, even though the nature of multi club ownership means you are actively running both.
