How do you deal with disc brakes and fenders?

We love our disc brakes, but they have made a mess of trying to attache fenders. So far in back we have been able to attach our old fender and rear bike rack, but only through the use of extra long bolts and hollow tubes to spring out the rear rack enough to clear the rear disc brake.
