Website or App to Track Food/Drink Consumed and Get Nutrtition Info?

Anyone know of a website (preferred) or app where one can input the food and drink one consumes and track/report on nutrition?

It doesn't have to be too much detail. Calories, carbs, fat, cholesterol, that sort of thing. Ideally it should graph it over time.

I'd just like to keep track of...
