It sacrifices 20% of mana per second to recover that much on your health pool until you reach full health. This effectively allows you to invest into Mana-regen as your main recovery method and it will sustain your mana and health. When building into this with a hierophant I quickly realized it makes Righetous Fire(40% more spell damage) easy to add into a build while still having good ehp sustain, and without RF on it feels as good as vaal pact overleech if not better in a lot of ways.
