A's action gets one stroke penalty (rule 9.4b) and A is required to replace and continue - only the latter part happened. When these facts are put before the Committee, it needs to verify whether the players were aware of or were uncertain about the penalty that they did not apply. If both are aware a penalty should have been applied, both are DQed (rule 1.3b(1). If A only is aware, A is DQed (rule 1.3b(1)). If B only is aware, the Committee may DQ B if it considers this was serious misconduct (rule 1.2a and 20.1(c(1)). If neither were aware, the Committee just adds the one stroke penalty to A's score (rule 3.3b(3)) unless it learns A was uncertain about a penalty and failed to check with the Committee - in which case A is DQed anyway (rule 3.3b(3)).
I would just add if only B knew, and is also A's marker, B should be DQ'd.    R3.3b(1)/1.
