2008 Trek Portland Hydraulic Disc Upgrade?

I have a 2008 Trek Portland. I have upgraded everything on the bike over the years. The one thing I'd love to do is convert the Avid BB7s to some Shimano Hydraulic discs. Has anyone else done this upgrade on a Trek Portland? The cable routing on that bike is not very traditional so I am trying to...
