Sorry if I arrive only now, regarding scenario 4 imo I would proceed according to the fact. And that is, what matters is the moment in which the player plays the original ball. Everything that happens afterwards, looking for the original ball, in my opinion is irrelevant for the ruling. According to the rules the player is not obliged to identify the ball before playing it, since if he doesn't play his ball, then he pays the consequences.
But in your opinion, the ball played to the green thinking it was his provisional, what ball would it be? Please don't tell me it's a wrong ball!
