I applaud your efforts in working through these hellishly complex situations that tend to occur more commonly in quizzes than on the golf course.  Don't beat yourself up too much with these, just relax and take them one step at a time.  On your new scenario, my ruling is as follows:

When the player lifted and dropped the stray ball he substituted a ball for his ball in play.  As soon as he played a stroke at that substituted ball, his original ball was lost and his provisional ball was his ball in play.  He has therefore substituted a ball for his provisional ball and played from a wrong place.  As this substituted ball is highly unlikely to be a serious breach, it is now his ball in play and he is required to finish the hole with the substituted ball.

Now to the accounting.  Where the provisional ball was lying, it was lying 3, comprising the original tee shot, the S&D penalty and the second tee shot.  When the player substituted from the wrong place, he incurred a further 2 shot penalty for playing from a wrong place (R13-1/R20-7) so he played his 6th shot.  The ball is in play and lying 6.

Some further observations:  whether the wrong place breach is serious is a critical issue.  IF IT WAS A SERIOUS BREACH (that is, if the position played from was significantly advantaged compared with the position of the provisional ball), the ruling changes because the substituted ball played from the wrong place would not be the ball in play and the player would be required to return to where the provisional ball was lifted and replace and play, incurring a further R18-2 one shot penalty for moving his ball in play.  The player would then be lying 6 and about to play his 7th.  This scenario is then very close to the scenario in D27-2b/9, with the small variation that it was a wrong ball there (because the player did not lift the stray but just played it) rather than a wrong place with serious breach.  Both of those situations demand that the next shot played be the provisional ball from its correct position, which brings on the additional one shot penalty because of the further error of moving that ball.
