Am I missing something here?  To me the problem is sort of obvious.  Since this is foursomes stroke play, and partners are required to alternate strokes until the ball is holed, B and his partner are DQed because they failed to hole out.  After player B putts and his ball misses the hole, his partner is required to complete the play of the hole.  When B back-handed the putt, the side was required to place the ball where it was stopped (+ the 2-stroke penalty) and then complete the play of the hole with B's partner making that last stroke.  The team teed off on the next hole without completing the play of the previous hole and thus are DQed.
