Can you identify this stem?

I'm guessing my bike is 15 years old or so.

I'm hoping someone is familiar with this particular stem. It has a yellow elastomer or urethane insert for shock absorption. As you can see in the pic, the material has degraded over the years (it used to fill most of the hole).
